Complete this form if you are a veteran under age 60, living in Texas, have little or no financial resources, and wish to apply for meal assistance through Meals for Vets, a program of Honor Veterans Now.

If a required document and/or requested information is not provided within two weeks of initial application, the request for assistance will become inactive. Meal assistance will not begin until all requirements are met and your application is approved.
