The CitySquare Community Health Services (CHS) Clinic serves as the "medical home" to over 2,000 patients, providing accessible and affordable preventative health care, chronic disease management and pharmacy benefits, to uninsured, low-income patients.
Partnerships with Baylor Scott and White and Health Texas Provider Network help the CHS provide medical care, referrals to its on-site diabetes management program and counseling.
